A community care assessment is an evaluation of your personal circumstances and needs, carried out by the social work department of your council. Social work will look at how you approach daily life, and recommend support or equipment that may make life easier for you. Might they suggest, for example:

Get some help with homework, such as cooking and housekeeping.
Moving to new accommodation where more assistance and support can be offered.
