A community care assessment is an evaluation of your personal circumstances and needs, carried out by the social work department of your council. Social work will look at how you approach daily life, and recommend support or equipment that may make life easier for you. Might they suggest, for example:
Anyone seeking assistance may get an evaluation of community care. Probably
Become disabled
Have a learning handicap
Having a mental health concern
Getting a chronic condition
Be opioid- or alcohol based
Be elderly.
Someone from the social work, housing or health department will carry out the test in order to:
Visit you and your family or caregiver (if appropriate) to
address your needs and what kind of assistance you want to
receive.
Find out what you can and can't do
If you agree, speak to your psychiatrist, occupational therapist or
other medical professionals.
The social work department should keep in touch with you whilst the assessment is being carried out.
