UTI- urinary tract infections is a infection of the urinary system and occur in the lower urinary tract.
Assessment
Subjective- painful urination, bloody or cloudy urine, changes in characteristic of urination, frequent night time urination.
Objective- inspect the inflammation of urinary meatus, palpate bladder distension or tenderness. Auscultation of renal arteries of bruits. Percussion of costovertebral angle for tenderness.
Diagnostic assessment-
Urine analysis for presence of puss cell , WBC, RBC or Bacteria.
BUN level - elevated
Serum creatinine level- elevated
Urinary retention - it is accumulation of the urine in the urinaary bladder. As bladder is unable to empty itself.
Risk factor assessment- BPH, disk surgery or hysterectomy and side effect of medication ( anticholinergic, antihistamines, antihypertensive, anesthetic agents).
Assessment-
Subjective- discomfort, anxiety, frequent urint and voiding, dribbling
Objective- assesment of degree of bladder is firm and distension, voiding frequency, discomfort or pain in pelvic region, overflow incontinence, fluid intake is larger than output.
