  1. The doctor orders 1 L of 0.9% sodium chloride q10h. The nurse starts the liter of IV at 0900. The IV infiltrates at 1100 and the nurse restarts the IV at 1200. Ampicillin l g in 50 mL D5W IVPB is given q12h at 1000 and 2200. Calculate the patient's 8 hr. parenteral intake for the 1500 - 2300 shift.

Expert Solution

ANS -

1) First fluid started is 0.9% sodium chloride q10h i.e. every 10hrly so it comes to be @ 100ml/hr from 09:00 am

2) Second fluid started is Ampicillin l g in 50 mL D5W IVPB is given q12h at 10:00 and 22:00

Q is to calculate patients 8hr fluid intake from 15:00 to 23:00.

NOTE - First fluid i.e. 0.9% sodium chloride was stopped from 11:00 as it infiltrated outside and was restarted at 12:00.

So now calculating for 15:00 to 23:00 time.

FLUID 1 is going on @ 100ml/hr from 09:00 am to 11:00 am then there is infiltration outside and is restarted at 12:00. @ 100ml/hr till 10 hours are completed.

so 9 to 11 are 2hours followed by restarted at 12pm so will have to continue for 8hrs more to make it as 10hrs, so will get over at 20:00

we need to calculate from 15:00 to 23:00 time so looking at this time slot 0.9% sodium chloride is running @ 100ml/hr from 15:00 to 20:00 - 100ml for 5hours = 500ml

FLUID 2 - Nurse is giving 50ml fluid at 10:00 and 22:00

We are just bothered about time between 15:00 to 23:00 so we will take 50ml fluid of 22:00 in concideration

SO TOTAL FLUID recived by patient between 15:00 to 23:00 is 500ml + 50ml = 550ml
