The Agency for Healthcare research and Quality main policy is Care Coordination which mainly involves in organizing patient care activities and sharing patient information among all physicians who involved with patient's care to achieve and provide safe and effective care to patient. It's main purporse is the awareness of patient needs and references which helps physicians providing quality of care to right patients at right time can improve outcomes of patient.

--Care coordination is very important role in the healthcare management due to mainly healthcare systems not often coordinated about patient's information between primary care sites and speciality sites. It leads to patients often do not have clarity that  why they are referred to specialists from primary care and what to do after that and also even specialists and primary care providers donot recieve full information about patient's treatment that leads to some confusion and takes time to make a decision. So Care Coordination plays crucial role in the provision of safer and high quality of care to the patient.

--This care coordination can be achieved by having teamwork, Care management, medication management , patient centered medical home and patient's Health information technology.
