The Agency for Healthcare research and Quality main policy is Care Coordination which mainly involves in organizing patient care activities and sharing patient information among all physicians who involved with patient's care to achieve and provide safe and effective care to patient. It's main purporse is the awareness of patient needs and references which helps physicians providing quality of care to right patients at right time can improve outcomes of patient.

--Care coordination is very important role in the healthcare management due to mainly healthcare systems not often coordinated about patient's information between primary care sites and speciality sites. It leads to patients often do not have clarity that  why they are referred to specialists from primary care and what to do after that and also even specialists and primary care providers donot recieve full information about patient's treatment that leads to some confusion and takes time to make a decision. So Care Coordination plays crucial role in the provision of safer and high quality of care to the patient.

--This care coordination can be achieved by having teamwork, Care management, medication management , patient centered medical home and patient's Health information technology.

Both the barriers and facilitators are identified in the Care coordinator policy in the work at organizational level, interpersonal level and individual level. both barriers and facilitators are involved in this policy are

--at organizational level, they are Functionality of clinical information technology like using Electronic health records for patient information ;

--The availability of community resources;

-at interpersonal level , Interactions with clinicians and other health care providers for patient health information in which poor communication leads to poor decision making which affects patient healthcare,

-- at individual level, selfcare practices for mental health and wellness.

Other barriers involved in this policy are excessive case loads and data mangament of population's health resposibilities .

A professional nurse shows leadership in different ways like promoting quality care for patient, improving patient outcomes and also influences on quality improvement activities of other nurses which leads to provide safe, effective and quality of care to the patient.

-- The nurse demonstrates leadership qualities by listening to the patient and their families about any concerns related to treatment and care and also helps them to make a decision related to treatment,

-- the professional nurse has awareness of knowledge on evidence based cases and offers valuable information and resources on understanding how medical errors can happen and follows safe protocols,

--The nursing leadership helps to improve patient outcomes, to reduce medical errors in patient healthcare, improving other nurse staff retention by inspiring them to work together in achieving quality of patient care,
