Hi, I'm doing a Client Teaching care plan. I listed the things I need below for the care plan

A non-compliant newly 35 y/o diagnosed diabetic patient, whose blood glucose levels are consistently above 250 D/L. The patient is obese at 274 lbs with a height of 5 feet 4 inches. The patient works at a fast food restaurant, single, has low income and has two small children. The patient states “I eat a lot at work, and I take extra food home for the kids, and I am not the best cook”.
1. Nursing Diagnosis
Type 2 Diabetes mellitus(Adult-onset diabetes or noninsulin-dependent diabetes)
-older than 30 years
-obesity
2. Client Goal/ evaluation
As the blood sugar levels are consistently high, the patient needs to know how to lower sugar levels and combat diabetes. The patient wants information about diabetes, what diet should be followed, or any change in lifestyle to improve his condition.
3. Teaching interventions for the patient ( 4 interventions).
You need to teach the patient about the facts of diabetes. It is a hormonal and metabolic disorder that can't be completely cured, but it can be treated and controlled.
The goals of managing diabetes are to improve Lifestyle patterns, healthy diet, regular exercise, and taking medications along with regular follow up is necessary for maintaining sugar level in diabetic patients.

Subjective Data:
This data is based on the subject's feelings and emotions. So, it may vary in different subjects.
