Joyce Workman, Joyce Workman, 42- year old female who presents to the Diabetes Clinic with a new diagnosis of type II diabetes. She has been documented as being obese, new onset hypertension, polyuria, and a rash on her abdomen. She was asymptomatic upon arrival. She was admitted yesterday for stabilization of her glucose levels, and assist her with lifestyle modification. She states she leads a sedentary lifestyle as a bank officer. Her HbA1C is 10%.
NURSING CARE PLAN:
SUBJECTIVE DATA:

Rash on her abdomen.

OBJECTIVE DATA:
High blood sugar levels.

NURSING DIAGNOSIS:
Polyuria secondary to diabetes mellitus.
Rash could be secondary to itching, other skin infections like dermatitis.
Risk for Cardiovascular diseases.
Risk for Urinary tract infections due to frequent urination.
Risk for developing arthritis due to excess weight.
NURSING INTERVENTIONS:
Monitor blood glucose levels before and after meals.
Monitor blood glucose levels.
Check peripheral pulses to look out for tissue perfusion.
Monitor for hypoglycemic symptoms like sweating, palpitations, confusion etc.
PATIENT EDUCATION:
Educate the patient regarding type II diabetes mellitus, hypertension and risks associated with them.
Instruct the patient to be physically active and to change the diet which helps in reducing weight.
Advise the patient to examine the feet thoroughly for cuts, sores, ulcers daily.
Advise the patient to strictly adhere to treatment and follow up visits.

To acheive optimum blood sugar and blood pressure.
To reduce weight.
To change sedantry to physically active lifestyle.
