1.acetaminophen (TYLENOL)

2. LORazepain (ATIVAN)

3. Heparin 5,000 units/mL injection

4. HydroCHLOROthiazide

5. Tamsulosin (FLOMAX )

Answer :

Drug 1. Acetaminophen :

* classification : these are non steroidal anti inflammatory drugs, these should be act like analgesics ( pain relievers) and antipyretics ( fever relievers).

* indications :

For minor pains,

Common cold,

Head ache,

Tooth ache,

Fever,

Minor pains in arthritis,

* adverse effects :

Nausea, stomach pain, loss of appetite, itching, rash, clay colored stool, dark urine ect.

* contraindications :

Caloric under nutrition, liver problems, liver failure, severe renal impairment, shock liver infection with hepatitis c virus.

* labs to review :

Liver function test should be done like total bilurubine, SGOT, SGPT ect.

Renal function test.

* special considerations :

Follow all rights of drugs administration.

Should not give more than 4 -5 days.

Assess and abseve for side effects and provide treatment.

Drug 2. Lorazapam  :

* classification : it is under classification of benzodiazepines,

It increases the effects of GABA receptors to work.

* general indications :

Anxiety disorders,

Stress

Depressive disorders associated with anxiety.

* Adverse effects :

Fatigue, drowsiness, amnesia, memory impairments, confusion, disorientation, depression, euphoria, suicidal attempts, ataxia, tremors, vertigo, seizures ect.

* contraindications :

Acute narrow angle glaucoma

Hyper sensitivity to benzodiazepines.

* labs to review :

RENAL FUNCTION TEST

liver function test

* special considerations :

Follow rights of drugs administration

Assess for adverse reactions

It can take by with or without food, or empty stomach.

Drug 3. Heparin :

* classification :

It is anticoagulant, and blood thinner,

* indications :

Deep venous thrombosis,

Pulmonary embolism,

Intravascular catheter occlusion prophylaxis

Cerebral thrombo embolism.

* side effects :

Easy bleeding,

Bruising,

Pain,

Redness,

Wormpth,

Irritation,

Bluish coloured skin or cyanosis ect.

* contraindications :

Uncontrollable active bleeding,

Disseminated intravascular coagulation,

Hypersensitive to heparin.

* labs to be reviewed :

Aptt, activated partial thromboplastine time should be monitor,

PT WITH INR also should be done.

So that we can know the bleeding disorders of client.

* special considerations :

It should be continue until INR with in therapeutic range for at least two days.

Monitoring blood tests like PT, APTT tests regularly during administration of heparin.

Drug. 4. Hydrochlorothiazide :

* classification : it is diuretics, increases the urine out put.

* indications :

Hypertension,

Congestive heart failure,

Symptomatic edema,

Diabetes insipidus,

Renal tubule acidosis ect.

* adverse effects :

Thirst, muscle weakness, constipation, blurred vision, dizziness, photosensitivity ect.

* contraindications :

Patients with anuria,

Acute and chronic renal insufficiency,

Hypersensitivity.

* labs to review :

Renal function test,

Serum electrolytes,

Blood glucose levels,

Blood urea nitrogen levels.

* special considerations :

Educate the patient regarding urine out put, so don't take at night time, it may distrurb for sleep.
