Chronic illness are diseases which continue for many years or till the end of life. Some of the chronic illness are hypertension, diabetes mellitus,arthritis,asthma,COPD,chronic kidney disease and psychiatric diseases like depression etc.These affect the people life and make some changes in people life.Certain illness can be treated but cannot be cured. Hence to maintain in a normal range individual need to take their daily medication dose.

Disability is not a disease it is a impairment.Either it can be physically or mentally.Also it can be congenital or acquires during life time.It reduces the individual participation in day to day life activities. It makes assistance to help them for performing activities of daily living.some of the disabilities are blindness,deafness,muscular dystrophy and other mental and intellectual disabilities like autism and down syndrome etc.Nowadays health care is providing rehabilitation services hence the disability cannot be cured but helps individual to perform their day to day activities.

If the disease is uncontrolled in chronic condition then it leads person to disability.
