For patients who have an acute disability or chronic diseases - does long term care would give the best and most effective care for these patients?
Long term care involves care for patients personally, socially, and medical services that assist people who have functional or cognitive limitations in their ability to perform self-care and other activities.The care is provided through different ways through private homes,adult-day care settings, and nursing homes.Integration of long-term and acute care in a managed care model serving a broad population may promote more effective acute care and more efficient acute care and more efficient and affordable long-term care.The nurse always emphasizes the patient's assets and strengths ,positively reinforcing his or her efforts to improve self-concept and self-care abilities .During nurse-patient interactions ,the nurse actively listens, encourages, and shares patient's successes.The nurse helps the patient to identify strengths and past successes and develop new goals. Coping with disability or chronic diseases ,self-care ,mobility,skin care, bowel and bladder management are frequently areas of nursing intervention.The nurse assumes as a role of patient advocate, caregiver,teacher,patient counselor,and consultant.
