explain how chronic illness imposes a burden on the health and quality of life of the older adult, and identify ways of preventing chronic illnesses. Each student should select a chronic illness to cover such as chronic obstructive pulmonary disease, heart failure, diabetes, kidney disease, etc and discuss the progression an older adult may face through the phases of nonfatal, serious, and frailty, using the nursing process. Include a teaching plan for the older adult and their significant others. Each student in the group should select a different chronic disease and support their writing with a minimum of 2 APA references, one of which may be the course text.

The older adults are at risk of get chronic diseases. Because of their altered immune system functioning and improper functioning of other body system. Most of the adults are suffer from chronic diseases such as chronic pulmonary obstruction, diabetes, kidney diseases, heart failure etc.

Here we are going to discuss about the diabetes condition in older adults. Diabetes means altered blood glucose level in blood sue to absence of insulin or inactivity of insulin in the body. Type 1 diabetes or diabetes due to lack of insulin production are condition occur from the childhood. Type2 diabetes are due to the insulin is produced from the pancrease normaly but it doesnot act against the glucose molecules present in the blood. This is a acuried condition due to aging and changes in the food content. During aging the pancrease secretory function become altered and the produced insulin may be inactive against the glucose particles.

The diabetes is a chronic condition and appropriate treatment and dietary modification is need to avoid the further complications. The complications of diabetes may lead to death and serious problems in old aged people. To prevent the diabetes older should be take appropriate diet and modify their lifestyle. The diet should not contain more sugar. Avoid excess surgar from diet. And also modification in lifestyle such as do walking or jogging to utilizing excessive glucose present in the blood by the cell. While doing activities body taken glucose to produce energy and it will reduce the blood glucose level.

If those methods donot prevent the diabetes then medical treatment is needed to control the diabetes. Antihyperglycemic drugs and insulin injections are recommended to reduce blood glucose level.
