Some abbreviations used in medication charting are considered high risk and can increase the likelihood of medication misadventure. State a high-risk abbreviation, outline the potential problems that may be associated with this abbreviation and give a safer alternative abbreviation.

By writting different ways can write same as medication .eg AC_before meals, it can be write as BM.
