compare the physical assessment of a child to that of an adult. in addition to describe the similar /different aspects of the physical assessment, explain how the nurse would offer instructions during the assessment how communication would be adapted to offer explanation and what strategies the nurse would use to encourage engagment
For a complete physical assessment the equipments are needed that are similar for both pediatric and adult client,there are some differences.
The pediatric client will require that the nurse use a neonatal ,infant and pediatric blood pressure cuff and techinques such as assessment of vital signs which vary among the age groups.Some of the reflexes present in the newborn are absent in the adults .
In adult clients head to toe sequence for assessing adult clients.
The sequence for the pediatric assessment generally altered to accomodate child's developmental needs .During head to toe assessment allow the children to touch or play with the equipment and ask the permission of the parents to change the clothing of the child or keeping the parent along with them.In case of adults privacy is provided while doing the physical assessment.
There should be communication with the parents is established by introducing self,ask how they want to be addressed , confidentiality,ensure privacy , direct focus and observe with cultural awareness ,use silence and empathy to avoid communication blocks and provide anticipatory guidance.
Main goals of paediatric assessment are to minimize stress and anxiety associated with assessment of various body parts,nurse -child parent relationship ,allow for maximum preparation of child and provide maximum accuracy of assessment findings.
When communicating with child the following should be kept in mind
Focus on child in your communication.
Explain them what,how, and why.
Should use words he or she will recognize.
Should be consistent.
When communicating with the adult the following should be kept in mind;
Should be honest with them.
Should be aware of privacy needs.
Think about the developmental regression.
Importance of peers.
Nurses should encourage the parents to talk and offers opportunities to determine about child helath and developmental status and other factors that may influence the child's life.
Allowing the parents time to talk about concerns they may have.Three things nurses can do to assist parents through anticipatory guidance to become more confident in their abilities are to base interventions on needs identification by family ,veiw the family as having the ability to be competent and provide opportunities for family to achieve competency.
When it comes to adults the way of communication is completely different . communication with adolescents is facilitated by encouraging expression of ideas and feelings respect their views ,tolerate differences , respect their privacy and set a good example.
