Medication reconciliation is the process of creating accurate list of possible of all medication of patient is taking inorder to prevent medication error and to prevent adverse effect. Medication reconciliation involves a three-step process: verification (collecting an accurate medication history); clarification (ensuring that the medications and doses are appropriate); and reconciliation (documenting every single change and making sure it “squares” with all the other medication information).
The most common problem facing by the patient in health care setting is lack of aware of medicines while transferring the patient from one setup to other. Medication reconcillation shoud start in the admission itself. The patient should take the list of previous medication to the hospital inorder to plan the next. Medicine shoul be reconcilled as soon as possible with in 24hours of hospital admission, it helpsvto further risk of medication and other medicine error. Patient have a good role in medicine reconcillation. Patient should bring previous history of medication inorder to give proper care. A standardise procees of medicine reconcillation helps to reduce the work of the nurses and pharmasists.
The three health care professions recognize the value of medication reconciliation and want to see it implemented in the hospital, yet there is a lack of agreement with regard to roles and responsibilities of each profession within the process. This needs to be addressed by the hospital administration to design clear procedures and defined roles for each profession within a standardized medication reconciliation process.the main obstacles in small healthcare setup may incude lack of knowledge regarding the medicines, improper communication and coordination between the professionals, patient lack of knowledge about the medication.this is acomplex and time consuming process and more professional are needed for a large hospital setup.
