Will there ever be a true medication reconciliation?
Yes, there will be medical reconciliation.
Medical reconciliation is the process of comparing patients medication orders to all of the medication the patient has been taking.
The reconciliation done to avoid medication errors such as omissions, adverse drug - drug disease drug interactions, drugdosage, drug timing, duplications etc ...
It should be done at every transition of care in which new medications are ordered or existing orders are rewritten.
Transition care includes changes in settings, services, practitioner's or level of care.
Dietary supplements are not currently considered many clinicals to be medications but as interaction occur with prescribed medications. medical reconciliation process considering dietary supplements and also all the medications supplements are part of the patient's medication history.
In the admission one unit (intensive care unit ) ordered new medication / discontinued medi action... If no medical reconciliation the patient going for another unit (general ward unit) rewritten of the same medication happens...
So medical reconciliation create a safe medical environment in acute care settings.
Omission of the drug the patient taking prior to admission checking by medical reconciliation process .
Reconciliation process requires verification with the patient's medications whether the medications originally prescribed by the pysician/ changed by another physician for cross checks in the outpatient records.
So medical reconciliation is important in hospital service.(admission and discharge services)
