A Mental Health Concern - and the Patient or Community - for your project.
A brief history of the patient including diagnoses and medications - or a brief description of the community issue.
Any substance abuse, addiction or violence issues surrounding this mental health problem. Describe the attempted interventions that have been made for your patient or community, and identify what has been successful and what has not. Submit a rough draft of information gathered so far.
Describe your thoughts regarding your patient's or community's mental health issue. Are there any cognitive concerns? Think about interventions that may be helpful. Include sources for evidence-based practice.
List appropriate nursing interventions for your chosen patient or community. How will you evaluate effectiveness? Include an evaluation tool or rubric.
Complete your Semester Project by identifying mental health resources that can be used for your chosen patient or community. Finish with an educational tool for your patient or community.

Introduction:
Nowadays, Most of the people are unable to cope up with the day-to-day situation in the life. They are frequently thinking and reacting in different ways. almost all the people were suffering from anxiety and depression. Mentally affected individuals feel unhealthy and lack of hope and confidence. This affects not only the individual but also the community as a whole. I like to discuss the public health issue of anxiety and depression.
Issues related to Anxiety and Depression:
Everyone experiencing some mental changes at some time. The most commonly seen in the community people are anxiety and depression. Anxiety is a state of fear, tensed or afraid of something. It is a normal behavior of the human being. But if it is prolonged, continuous, and affects the health of the person, then it is diagnosed as an anxiety disorder. The affected persons will show the symptoms of sleep disturbances, fear of life, panic, headache, irritability, loss of appetite, and annoyed. Some may have a generalized disorder, some may have social anxiety, or panic anxiety and the most commonly seen post-traumatic stress disorder.
Anxiety changed into a state of depression if not treated. Depression is a condition where the individual feels isolated, hopelessness, worthless, guilty, and exhausted. It affects the normal living state of the person and the individual thought of suicidal action.
The CDC considered the depression as a serious public health issue. After heart disease, depression is considered as a second most common cause for disability around the world. It not only affects the individual, but also the friends, families, colleagues, and the communities. Identifying the person and treating and providing services is challengeable in the community. The anxiety and depression were togetherly seen in substance abuse patients.
Attempted Interventions:
Prevention of suicide at community levels by identifying the people, data collecting, and providing education for suicide prevention. WHO also increasing mental health services and making awareness through social media. Public Health Organization is making awareness, improving treatment, and prevention of this public health issues. These interventions can control the condition, simultaneously it exceeds.
Cognitive concerns:
Depression is usually associated with the anxiety. This patient is usually affected with mild cognitive impairment as the age increases. It gives problem in memory, language, judgment, and thought. These individuals are more prone to be affected by dementia and Alzheimer's disease. They usually have the symptoms of loss of appetite, sleep problems, restlessness, irritability, isolation. This can be treated at the early stage and gives more importance to prevention.

An evaluation must be done after proper interventions and also for assessing the client current mental health status. Various tools of evaluation can be used to evaluate the client.

If the patient is getting a score of 9 or more shows positive for anxiety disorder. Scoring 3 or less is considered normal. Between 4 and 8 has negative reflective value.
