Chronic disease is a condition which is long lasting in nature which are non communicable disease such as diabetes, heart disease, cancer etc. Chronic diseases are the leading cause of death in whole world.

Chronic disease is also called life style disease and it can be prevented by life style modifications. Life style modifications enhances the importance of self care. Strategies to promote self care for patient with chronic disease are the following :

  • Eat well balanced diet in which include fruits and vegetables, avoid junk food, processed and packed food.
  • Regular exercise
  • Develop regular sleep routine
  • Avoid tobacco and alcoholism
  • Do meditation and yoga to reduce stress
  • Drink adequate water
  • Regular follow up
  • Right medication on right time

An intervention package provided the patient with chronic disease. This package include the teaching program regarding life style modifications and demonstration of exerciseand also regarding the disease . It can be provided to group of individual with same chronic disease. This can be reduce the progression of disease.
