Medication type:Levothyroixine(Levothyroid)

Category:Thyroid supplement

Pharmacological action:It exerts their physiological action by controlling of DNA transcription and protein synthesis.Triiodothyroxine and levothyroxine diffuse into nucleus that bind to the thyroid receptors that is attached to the DNA.The physiological action is to produce T3 from the T4 by deiodination in the peripheral tissues.

Therapeutic use:

Hypothyroidism
Myxedema coma
Thyroid stimulating hormone suppression therapy

Complications (Adverse effects):It is caused due to overdose which includes:

Central nervous system: Hyperactivity, anxiety, irritability, insomnia
Musculoskeletal:Tremors.muscle weakness
Cardiac:Arrhythmias,Hypertension,angina,heart failure
Pulmonary:Dyspnea
Gastrointestinal:Diarrhea,abdominal cramps
Reproductive:Impaired fertility

Medication administration: Intravenous.Oral

Contraindications:

In Pregnancy and lactation mothers
For patients who have thyroxicosis
Myocardial Infarction
Patients who have cardiac problems
Cautious for obese people

Nursing Interventions:

Obtain vital signs,weight,height and monitor vitals through out the treatment,
Initial therapy start from low dose that increase gradually over several weeks.
Monitor thyroid level

Interactions:

Sucralfate reduce the absorption with concurrent use with levothyroxine.
When Antidepressants and levothyroxine take concurrently that can increase metabolism of levothyroxine
Levothyroxine can increase the anticoagulant effect by breaking the vitamin K

Evaluation of medication effectiveness:

Depending on therapeutic intent,effectiveness may include:Thyroid stimulating hormone and T3 are expected within normal range.
Decrease or absence of symptoms such as weight gain, cold intolerance, dryskin, bradycardia.

Patient Education:

Advise patients if any side effects was seen during therapy.
Instruct patient to take daily dose on an empty stomach.
Provide patient education about the medication.
Advise not to discontinue the medication without consulting doctor.
