   Health care cost for the average family are going up..it was doubled in the past decads..government is likely to try to medicare and medicaid spending,but it is unlikely that there will be and oveall limit placed on health care spending..business and government asking the public to pay more out of pockets for their health insurance and the care they receive..
   The gap between researchers and practitioners extends to the professional societies..The gap between research and practice is fundamental..Knowledge and skills needs each different group..in community area it is called design research..This community find out the problem and difficulties among human and social behavior..This far easier method to get benefit and changes in work and life patterns..but also because new technologies are mostly accompanied by problems..
   The research-practice gap is only one of many problems facing the design profession..most of the beliefs in the practice od design found many of the principles..the real problem is believe we do that are based more on faith than on data..researchers in many studies comparing individuals with groups,or people in one culture with those of another..
   Health care reform legislation need to address promotion of local hospital medical practice,incentives to practice in rural and local ares and recruitement and education for all forms of local health care professionals..medical providers are increasingly employing health information technology in local hospital to improve patient safety,quality of care and efficiency..Reducing readmissions is a national priority for payers,providers and policy makers seeking to acheive triple aim objectives of improved health and enhanced care at lower cost..
