A 74 year old woman, with a history of hypertension and Urinary bladder prolapse was examined and urine culture found positive for E coli. She was started on nitrofurantoin 100 mg oral twice daily. Seven hours later she developed shaking chills with fever up to 102.70 F, tachypnea and tachycardia. Her heart rate also was up to 120beats/min.
