Explain the QSEN competency Patient Centered Care and two nursing interventions that are in this category and why.
QSEN
It is a national initiative to identify the competencies and knowledge, skills and attitude needed by all nurses to continuously improve the quality safety of health care.
It is used to prepare the nurses for future challenges with knowledge and skills to improve quality and safety of healthcare system.
It includes 7 elements
1. Patient centered care
2. Teamwork and collaboration
3. Evidence based practice
4. Quality improvement and safety
5. Informatics and technology
6. System based practice
7. Leadership and professionalism
PATIENT CENTERED CARE
The patient or designee as the source of control full partner in providing compassionate and coordinated care based on respect for patient value, attitude, needs and preference.
Nurse should understand the patient attitude, background, values, community and culture in order to perform the patient centered care.
Principles of patient centered care
1. Respects for patient values, preference and expressed need
Care should be providing in the atmosphere that is respectful to individual patient and focus on quality of life issues.
2. Coordination and integration of care
Coordination and integration of clinical care, support services and front-line patient care
3. Information and education
Proper information about patient clinical status, treatment, prognosis and progress
It promotes autonomy, self-care and promotion of health.
4. Physical comfort
Providing assistance in physical needs, ensuring patient needs for privacy. Patient accommodation area should be clean, comfortable and appropriate accessible by family members
5. Emotional support and alleviation of anxiety
Nurses should consider the fear related to treatment, prognosis and illness
Anxiety over impact of illness on family and financial impact on illness.
6. Involvement of family and friends
Recognition of family need, accommodation of patient family or friend involve in emotional and social support by clinician.
7. Continuity and transition
Giving information about social, financial, physical support for continuity of care.
Detailed and understandable information about medication, physical limitation and dietary needs etc. proper discharge plant of the client. Scheduling appointment and access to transportation for visit. Clear information when and how referral system works.
Skills and attitude – nurse should be able to elicit the patient needs, preference as a part of care. Plant according and implement it.
Care should be provided in a manner that sensitivity and respect for diversity of human experience maintained.
Assess therapeutic boundaries and facilitate patient care consent
Engage patient in self-care, involvement of family members or loved ones.
Resolve conflict in care, assess patient discomfort, removing barrier in patient care.
She should respect the patient value, and willingly support the patient centered care and groups.
Nursing intervention in patient centered care
1. Planning care for client – nurse should involve client while planning care or doing intervention to promote the healthy environment.
Why- by doing this client will sense of autonomy and feels his opinion matter for care provider.
For example- planning a diet for client, asking preference, belief related to diet will help to adherence with diet plan.
2. Removing disturbing element – physical comfort is very important for a patient for his or her treatment. Noisy environment, bright light, and clean environment promote client health and sound sleep. It also promotes patient safety
Why- by implement this intervention help a client for mental stability and clean environment always promote health and healthy enviroment
