In simple words patient centered care can be defined as care
which mainly focuses patient, their needs, values and concerns. The
goal is to achieve overall well being of the patient physically,
mentally, socially, economically and spiritually. The achievement
of this goal is not only the target of nursing professionals but
also every healthcare institution that deals with 'Patients'. This
patient centered care in professional nursing has various
characteristics:
- Patient should be considered as a unique
person but care should be given without any
discrimination of age,gender, caste, nationality or
culture.
- Respect to patient's preferences, needs, beliefs and
values.
- Build a good rapport and trust with the
patient. Use be proper channels and methods of communication is
important.
- Involvement of family members and friends who
is trustworthy to them in treatment regimen. This will make
patients more cooperative and understanding to physicians
instruction and the treatment course.
- Provide information and education to patients
about their condition and treatment. Transparency between the
patient and care provider is must. Information should be correct
and truthful. This will help them to make correct decision.
- Opportunity to make decisions for their care,
participate in treatment plan and make choices.
- Care should be accessible to patient.
- Care should focus on physical comfort as well
as provide emotional support when needed.
- Coordination and integration of care is
important between patient and care providers and also between the
team members.
- The care provided should be continuous. Transition of
care may also be required at times.
- Patient safety should be a major concern of
the care like preventing patient falls, adequate hand hygiene.
- Patient's right to privacy and confidentiality
