What are some health care system barriers to achieving the seamless continuum? of long-term care systems to allow the elderly to have access and afford so that they may remain in our community.

Barriers to achieving Seamless Continuum:
To provide care to the needed elderly at all possible levels is referred to as continuum of care. The aged people of community needs the utmost care with delicacy in order to lead a better life, But to achieve this, there are broad range of challenges that comes into play. For instance, it is required for the health care professionals, physiotherapists and people working seperately to consider the ecomomic status of an elderly. Instead of different views there should be a common treatment that must be carried out by group of professionals. Lack of skilled professionals, unavailability of diagnostic instruments, high cost of healthcare makes a challenge among older individuals to secure proper healthcare.
Social, Family or “non-system” barriers:
A support from family is what most needed for older individuals. Most people do not receive such care,even if they receive it becomes very late. This hampers medication procedure. Another major challenge faced by older individuals is the language barrier. A wide gap in communication makes a great difficulty in achieving proper healthcare.
