nursing care plan for urinary elimination
- Recommend adequate intake of water(2-3liter). Avoid caffeine intake , take Vitamin C . Sufficient hydration helps in preventing urinary infection and helps in excretion of sulfa drugs
- observe for any cloudy or bloody urine , or if there is any foul smell in urine because this can indicate towards infection of urinary tract or kidney.
- administer drugs which reduces bladder spasticity and decreases symptoms of frequency, urgency, nocturia ( oxybutynine , propantheline, hyoscyamine)
- use of Catheter can help in elimination of urine if patient is suffering from urine retention or incontinence. Monitor creatinine and WBC count.
- Periodic testing is necessary, if count is more than 1 lakh this indicates infection.
- If patient is having UTI, antibiotics are given. ( Bacteriostatic agents - Nitrofurantoin, cotrimoxazole , Ciprofloxacin, norfloxacin )
- patient should properly clean his hands and gential region
