Answer. In case of urinary inconsistance, there is problem with controlling the bladder resulting in involuntary lose of urine. The care plans for this problem are:-

. Making a clear path and removing barriers from the way to toilet.

. Proper lights to be adjusted when the pat2has to go to toilet during night.

. By setting a toilet schedule, like after every 2 hours they have to go to pass urine. With this, the body's system adjust accordingly.

. Clothing is also a barrier sometimes. So, clothing with proper ease to reach the toilet when needed should be considered.

. Extra or additional help in providing toilet seat for the patient besides the comode to ease their reach.

. Decreasing the fluid intake to minimal required amounts.

. Setting alarms, etc to keep the patient aware of going to toilet in fixed time.

. Educating the patient and the family about the importance of prompting voiding timely, to set body's system.

* In Case Of Bowel or fecal inconsistance, there is problem with the control in passing the fecal matter, that it leaks from rectum unexpectedly. The nursing care plans for this inconsistance are:-

. With the consent of a dietician, high fibre diet should be given to the patient.

. Fluid intake should be monitored according to the health.

. Providing extra accessible comode nearby for the ease of patient.

. Increased intake of natural bulking agents to thicken stools, for example, foods such as banana, rice, and yogurt.

. Creating a fixed or routine based bowel elimination time to set body's system.

. The patient should be discouraged to use pads or diapers for longer periods to worsen the problem.

. For some critical patients the external or internal stool pouches can be administered for the ease of patients.
