Once upon a time mary went to outside with her friend liza, suddenly Lizza developed shortness had syncope and she fainted.she doesn't have consciousness. Mary checked for her pulse, her pulse are irregular and weak. Her carotid pulse are present. Mary brought her to the hospital. On the way Lizza got consciousness. In the emergency department, doctor advised for an ecg, vital sign checked. Her heart rate was below 35/mt, blood pressure was 80/60.her grbs was 250mg/dl.she is a known case of diabetes mellitus and coronary artery disease on medication aspirin , also had stroke in the last year.that time she got leftside paresis.. Ecg showed complete heart block. Liza was very fatigue and hadchest pain, shortness breath and some confusions.. Doctor advised for cardiology consultation. The cardiologist came and done echocardiogram, reviewed the ecg,the cardiolgist said to marry about the heart block, that is No electrical messages get through ms.liza's heart. The rate and rhythm of her heartbeat is very slow or it may even stop entirely. This type of heart block can be fatal.so immediately need temporary pacemaker implantation before doing permanent pacemaker.because she is taking aspirin so it is to be stopped before surgery (before ppl)., because there is complication of hemorrhage. mary agreed forthat by discussing with liza's family.doctor obtained consent for pacing and virology tests,.and before that explained the procedure to Liza.doctor prescribed for an antibiotics, advised to do preparation of patientand for laboratory test of hemoglobin. .patient shifted to cath lab for temporary pacing.after that her heart rate improved, her symptoms reduced.chest xray taken to confirm the pacing lead.ecg showed normal sinus rhythm.

After 5days of stoppage of aspirin.she planned for permanent pacemaker implantation.before that cardiologist seeks advise from the neurologist and anaesthetist.ms.Liza said to neurologist that she had one episode of seizure in last month.he advised for an eeg.eeg done it shows no electical brain activities of seizure.anaesthetic evaluated her for any history of COPD, or obstructive lung diseases.no evidence for that. Both gave fitness.pacemaker implantation done.now Liza feels better.after 5days of antibiotics, she got discharged from hospital.the nurse and doctors advised regular follow up, said for glycemic control.she seeked a consultation from diabetologist also.now she is very happy with her family.
