Most American doctors have been trained in a fee-for-service cost unconscious culture. A serious expenditure containment strategy must address issues of utilization of services. Many studies of appropriateness suggest large amounts of inappropriate hospitalization and surgery. Price controls or negotiated price reductions on doctors and hospitals alone, without addressing actual practice patterns, won't come close to solving the problem of expenditure growth.
