Many physicians surveyed believe managed care has significant negative effects on the physician-patient relationship, the ability to carry out ethical obligations, and on quality of patient care. These results have implications for health care system reform efforts.Managed care" is a global term for health care systems that integrate the delivery and financing of health care. Managed care contrasts with liberal medical practice, which allows doctors to make clinical decisions and bill for their services without interference from managers or payers.
It was or is inevitable that serious changes to contain the growth of health expenditures would be bad news for many doctors, including some loss of income and autonomy. Most American doctors have been trained in a fee-for-service cost unconscious culture. A serious expenditure containment strategy must address issues of utilization of services. Many studies of appropriateness suggest large amounts of inappropriate hospitalization and surgery. Price controls or negotiated price reductions on doctors and hospitals alone, without addressing actual practice patterns, won't come close to solving the problem of expenditure growth.
