population health is health of a group of individuals , including the distribution of such outcomes within the group. It is an art towards equal health care delivery to an anticipiated groups of people in a particular geographical location

according to WHO , health is a state of complete physical , mental ,and social well being and not merly absence of disease or infirmity

one methode to improove population health is Population health managment (PHM) , defined as the technical field of endeavor , which utilizes variety of individuals , organizational and cultural interventioms to iimproove the morbidity (injury and illness burden) pattern and health care use of defined populations.

PHM distingush from disease managment by including more chronic condition and disease, by use of a single point of contact and cordination, . PHM includes those at lower level of predictable health risk.

components of PHM are

1 . Data Aggregation

a data warhouse is required to collect member informations from the many data sources (clinical , behavioral , administrative , patient records ...) and organizations (speciality hospital , multispeciality physicians , small clinics ,long term care , home health ... )

2. Patient Stratification

the purpose of this stratification is to identify the health needs of individual in the population such as complex care , chronic disease , peoples with a rising risk of disease,...

3. Care cordination (care managment)

this most oftebn focussed on top five percentage of people who accounts for nearly fifty percentage of health care costs.

4. Patient engagment

this requires care providers and community partners to acively create personal relationship with patients . this can be by 24 hr helpline , extended office hours , creating mobile apps that connect patients , care providers..

5 . Performance reporting

monitor the measures at the individual provider and patient level , but also need for analytics to figure out why something is not improoving , whether its protocol , people or training , which fault.

6.Administrative / business

managing and administering the magnitude of new value based model is something totaly new for providers.
