case management system is a care delivery model designed to
coordinate and manage patient care across the continum of health
care system.It is a dynamic and systematic collaborative approach
to provide and coordinate health care services to a defined
population.
The four components of case management system are;
- intake; which means the first meeting of a case manager with a
new client and taking informations regarding their health, problems
and identifying the key repsonsibility of a nurse to that patient.
This first meeting helps the case manager to identify how the
patient can receive the best care while they are at the health care
facility.
- need assessment;builds on the information collected during the
intake.Here, the case manager identfies the problems. analyse it
and check any risks to success.
- service planning;it is important to thw success or failure of
the client.The case manager establish specific goals and actions to
meet this goals.it should be both achievable and measurable.
- monitoring and evaluation; based on the informations availavble
through the previous stages,the case manager should continuosly
monitor an evaluate the cleints progress.continuous monitoring will
help to achieve the desired outcomes.
