Prior to referring, you will ensure that:
1. Discussions have been held with your supervisor or unit
manager,
and the client, their family/ carer or support team.
2. Discussions have been held with current support providers and
that
all possible alternative care options have been explored.
When completing the referral, you will ensure that as much
information as
possible is included and make sure you explain why there is a need
to refer at
this time.
Prior to accepting the referral for a younger person with a
disability, the
supervisor will confirm in writing that all other service options
have been
demonstrably exhausted. The confirmation letter will be signed by
the
Manager and include:
▪ A statement about the options that have been explored and
the
reasons these are not suitable or not available.
▪ The reasons why additional needs can or should not be
addressed
independently
You will complete referrals in 10 font and send them in accordance
with the
policy relating to email.
You will maintain the referral by using the expiry date 12 months
from the
date of the referral and review the client each month to determine
whether
additional requirements should be added.
Additional referrals must refer to the original document and stored
in the
client file on the organisational shared drive.

1. In order to complete a referral in accordance with the protocol, we need to ensure that complete discussions with details have been held with the supervisor or the unit manager, the client, their involved family or the support team. Additionally, detailed discussions should be carried out with the current support providers and ensure all alternative options for treatment have been explored and evaluated. All information pertaining to the case must be collated and proper explanation must be provided for the reason for referral.
2. For the maintenance of a referral in accordance with the protocol, the supervisor needs to give a written confirmation that all service options have been explored and no other option is left. In order to accept all referrals for a younger person with disability, the letter must by duly ratified by the manager with an explanation of all the options explored and why it turned out to be unsuitable or not available. It should also specifically mention, the reasons as to why additional needs can or should not be addressed independently. The desired font size would be 10 and sent with the policy relating to the email.
3. For storing the referral in accordance with the protocol, an expiry date of 12 months from the date of referral must be maintained and tracked. The client should be reviewed every month to ensure that any additional requirements are added. All these additional referrals must be mapped to the original document and stored in the client file on the shared drive of the organization.
