Describe abnormal assessment findings that occur with lower neuron dysfunction of cranial nerves V and VII. Also describe what sort of problems might a client with these deficits have?
As a general rule whenever the Lower motor neuron function is affected then single group of Muscles are affected ,tone power is Decreased leading to Decreased movement on that side also there is flaccidity and the superficial ,deep reflexes are lost and Babinski sign is negative .
For Trigeminal nerve 5 th nerve dysfunction of lower motor neuron ---
Follwing findings are seen
1)Total loss of sensations over one side of face over the divisons of Trigeminal nerve
2)Trophic ulcers in mouth ,nose and cornea
3)weakness of Muscles of mastication ( Masseter,temporalis and pterygoid )
4) Deviation of jaw to one side
5 ) Jaw jerk is absent on side of nerve damage .
6) Trigeminal neuralgias pain on same side of face of affected nerve.
Lower motor neuron damage of FACIAL NERVE 7 th nerve.
Lesion - At or below the level of nucleus upto myoneural junction.
1) One half of face is involved ( Upper and lower )
2) All the movements on affected side is lost.
So if lest sided LMN palsy is there so loss of nasolabial folds ,loss of forhead wrinkling ,unable to whistle ,drooling of saliva from left side of mouth and angle of mouth on right side is deviated classical findings seen in facial nerve palsy .
3) Bells phenomenon is present
4) Fasciculation or muscle atrophy on affected side
5) Taste sensation is lost
6) loss of corneal reflex
7) Hyperacusis if the nerve to sapedius is affected
8) Planter is Flexor ( Babinski sign is negative )
9) LMN may be isolated phenomenon ( Bells palsy ) or associated with Hemiplegia
Bells palsy Is isolated lower motor neuron palsy of Seventh nerve in which sensations on one side of face is affected and Pateint is unable to close the eyes on affected side .Attempting to close the eyes is rotated outward hence Bells phenomenon .So eyes are open so exposure keratitis can occur hence tear drops are prescribed .
