A synapse is the junction where two neurones meet and transfer signal from one to the other.
In a synapse a axon arrives with a signal. The axon terminal of that axon becomes the presynaptic membrane. The neurotransmitter is related from the presynaptic membrane by the fusion of vescicles carrying neurotransmitter to the presynaptic membrane. The neurotransmitter now reach the synaptic cleft. From there the neurotransmitter binds to the receptor on the motor end plate of the post synaptic memberane of the adjacent axon. The post sympathetic membrane is the dendrites of the new axon. The signal flows from presynaptic membrane to post synaptic memberane.
