Book expresses about the various dimensions of accidental analysis and preventive perceptions based on possible mechanism of accidents.
Contents of book includes six chapters:
1) Accidents and causes: includes constituents of accident,narrow misses,causes and consequenses. unexpected events and outcome,facts and explanation comparisions. from technological failure to human error, relative cause rather than nature od cause.
2) Thinking about accidents:includes accident models and its need.accident model is classified in three tyyes - a) sequential model- indicating root cause of accident. -b) Epidemiological model-indicating performance deviations, environment conditions,focus on barriers and latent conditions. -c) Systemic model -indicating sharp end to blunt end factor for development of accidental model. its consequence by this model.
3) Barrier functions and Barrier Systems: includes barrier origin, how barrier works,barrier locations. Barriers are classified into- physical, functional, symbolic,incorporeal (regulations). Each barrier system has various possible functions. Barrier design and its design effective failure modes and analysis is detailes in this chapter.
4) Understanding the role of barriers in accidents: represents role of barrier in accidental analysis. This shows accidental analysis in graphical presentation such as fault trees, event trees its comparision with respect to AEB model and variation diagram with structured analysis diagram and its limitations. Barrier functions are explained in terms of reciprocity(dependence), barrier bypass, unintended barriers and bi- directional barriers,
5) A systemic accident model: focus on time and variability, it focus on normal (ideal) or actuaol condition of work. the efficiency thoroughness tradeness is explined in depth. . ETTO rules considering level of cognitive functioning, level of individual work and organizational level vs actual practice is eaplained. stochastic resonance consideration for complex system to fuctional resonance in accidental model is elaborated.
6) Accident prevention: includes risk analsis, imagination based on facts ,requisite and harness. Accidental prediction based on four steps- 1) Identifying essential sytem function. 2) potential of variability. 3) Functional resonance based on expected and unexpected connections. 4) finalizing on countermeasures.. it also includes detection and control of performance variability.
This book offers framework for analysing accidents which helps in better understanding to prevent them.it provides effective sense to respond in desired manner.. effective study supports in minimising the number and diminution of consequences.this book illustrates reduction opf accident and its consequences based on concept of barriers in combition with systemic view of accident as effective means.
