Care Management--- It involves to improve the healthcare status of patients by increasing facilities , quality of health, interventions, by decreasing visit of clients to hospital, so to get effective outcome.

Care management is specific to individual, client centred, and all stages of care and treatment is managed properly wheras case management involves the care in coordinated manner like as whole rehabilitation is done, client has apportunity to use varities of health services.
