Ans. Bipolar Disorder:- Bipolar disorder is a type of mental illness which is marked by extreme shifts in the mood. The Symptoms of Bioplar disorder include an extremely elevated mood of the patient which is called mania & the patient can also include episodes of depression. So Bipolar disorder is also known as manic depression.

Diagnosis:- Risk for self-directed violence due to disease condition, risk of injury.

S- Subjective data :-

Patient/family states that he/she is not doing good. Sometimes he/she feel the symptom of Depressive he feels down at times, want to stay alone, don’t want to talk anyone. He/she has suicide plan and has thought about suicide. He/she also states that sometime he/she feel excited to do the things, take little sleep, talk unnecessarily, don’t able to focus on one thing.

O- Objective data

Vitals sign: temperature 98.4, Pulse- 82, Respiration- 16, Blood Pressure- 122/78mmhg

General assessment: Disoriented to person, place, and time. Affect is significantly effected due to range of emotional expression. Laboratory findings: CBC normal and Thyroid function test -TSH in low limit shows hypothyroidism.

A- Assessment

Patient is experiencing both the depressive & mania symptoms. Patient diagnosed as Bipolar disorder.

P- Planning

· Plan to reduce the stimuli in client’s surrounding environment which alter their mood like loud noises, bright light. Try to use short, simple and brief explanations while communicating with the patient. When the patient was less manic try to make them join one or two other clients in quiet, non stimulating activities like drawing, board games, cards. Administer the Lithium (mood stabilizeter), it is the first-line choice medication for preventing mood instability and also help to treating mania among the patient. Antipsychotics like olanzapine (Zyprexa), risperidone (Risperdal) for the patient is having symptoms of depression or mania. Antidepressant-antipsychotic drugs used these are the medication like Symbyax which is combines with the antidepressant fluoxetine and the antipsychotic olanzapine.
