Scenario:
The RN is developing a teaching plan for a male patient with a new diagnosis of peptic ulcer disease related to an H. pylori infection.
The patient immigrated to the United States from Vietnam two years ago with his extended family. He denies any current use of alcohol but has smoked one pack of cigarettes per day for the last 20 years. He reports increased fatigue during exercise, uses Chinese medicine to manage frequent muscle pain and has noted an occasional black bowel movement.

? 1, Helicobactor pyloric(H.pyloric)is a type of
bacteria..This germs can enter into our body and go to digestive
system.It will present in the tract for many years and causes
sores(ulcer) in a stomach and small intestines..Most of the people
will have this H.pylori in his body..They don't realize because
they never get sick..when you people have signs ans symptoms like
burning and pain in the stomach that increases when the stomach is
empty,nausea,loss of appitite.burping,bloating,weight loss.symptoms
worse when you have bloody or black tarry stools..
? 2, Exact causes still unknown for this disease..but
it can spread through person to person from saliva,vomit or fecal
matter and also through contaminated food and water..
? 3, Prevention: H.pylori will be presented from
childhood,Living in crowded condition and living without clean
water and contaminated dirty water by drinking the risk for
H.pylori..when people having advanced stomach ulcer with this
infection they may develop with stomach cancer..so early treatment
and screening is neccessary..
Cultural consideration:-
? As they shifted from vietnam they will follow lot of cultural
beliefs..so assess patient beliefs and attitudes and find out what
differnces they feel here culturally..Explain the cultural
regulation in america with good eye contact and smile .this should
be considered as a response..
? As they going to follow the western treatment explain it will be
different from vietnam treatment..In vietnam culture they lived
with their entended family..As they moved to america the living
style is entirely different independent country..because the
patient got this peptic ulcer and H.pyloric infection advice them
to avoid over crowded in this situation..Advice as per american
culture close contact between people must be avoided to prevent the
hihg risk..
Social worker:
? social worker is a person will collaborate with patient and
family as a counsellor and they will interact with them for good
communication..RN will explain everything about disease and
treatement but social worker will fill the gap of service what the
RN left with patient..They also be as a direct collaborator and
communication person for family and patient..He can explain about
socio-economic background for patient and how his smoking habits
will affect the patient health and 10% of the american male
population experience with peptic ulcer.how unlimited number of
people can affected from this disease..social worker acts as a
mediater for family and health care personals..
