What are the interventions and relationships between advanced practice nurses and families in creating positive outcomes in palliative and end-of-life care
Nurses are obligated to provide care that includes the promotion of comfort, relief of pain and other symptoms, and support for patients, families and others close to the patient.decision making for the end of patients life should occur over years rather than just in the minutes or days before a patients death.
During end- of- life care the nursing care planning revolves around controlling pain,preventing or managing complications,maintaining quality of life as possible, and planning in place to meet patients and or family last wishes.
But avoiding suffering,having your end- of- life wishes followed and being treated with respect while dying are common hopes.generally speaking people who are dying need care in four areas physical comfort,mental and emotional needs,spiritual issues,and practical tasks.their families need support as well.
* identify resources within themselves CTO sea with situation.
* visit regularly and participate positively in care of patient within limits of abilities..
* express more realistic understanding and expectations of patient
* provide opportunity for patient to deal with situation in own way
* assess level of anxiety present in family
* Establish rapport and acknowledge difficulty of the situation for the family.
* determine the level of impairment of perceptual cognitive,and or physical abilities.evaluate illness and current behaviors that are interfering with the Care in the patient.
* discuss underlying reasons for patient behaviors with family.
End of life and palliative Care helpes improve the quality of life for someone who has a life limiting illness by offering service, advice,information,referral and support.end of life and palliative Care offers emotional and practical support to families,friends and carers.
The role of palliative care at the end of life is to relieve the suffering of patients and their families by the comprehensive assessment and treatment of physical,psychological and spiritual symptoms patients experience.as comfort measures intensify,so does the support provided to dying patients family.
