Rapid assessment vs routine assessment.

Levels of prevention; primary, secondary and tertiary.

Terms: urticaria, pruritis, inflammation, edema, tachycardia, bradycardia, tachypnea, bradypnea, hyperventilation, hypoventilation, nocturia, hematuria, dysuria, anuria

Assessment techniques: Inspection, palpation, percussion and auscultation

While collecting health history ask :-

Name, Age, Gender,DOB, Providers( Referring Provider, Primary Care Provider), ASk the reason for why they are here today.

PAST AND PRESENT MEDICAL PROBLEMS:- High blood pressure, heart attack,high cholestrol,cancer,clotting disorders,diabetes, kidney disorders,thyroid deseae. Other past histories not listed.

SURGICAL HISTORY:- Any surgeries done in the past.

Do you drink alcohol? how often? How many per week?

Do you use recreational drugs?

Have you ever used tobacco? How many you use evryday and how often?

How many years have you smoke? When did you quit?

FAMILY HISTORY:-Any family history of heart desease, bleeding disordes,high blood pressure,blood clots,cancer, high cholestrol, stroke, diabetes.

CURRENT MEDICATIONS OR ALLERGIES:-

Do you have any known allergies to medications? Mention the medicines.

What is your current weight? height?

List out the medications you are currently taking.

REVIEW OF SYSTEMS:-

Constitutional:- fatigue/ night sweats,fever/chills,general health excellent/poor,unexplained weight loss/gain.

Eyes:- blurred vision/double vision, cataract, glasses/contact lenses/ blindness. glaucoma/ retinopathy, partialloss of vision / blind spots.

Ears/ nose /mouth/throat:- dentures/ difficulty in swallowing, hearing loss/ringing in ears, prolonged nose bleeds, voice change.

Cardiovascular:- ankle swelling/varicosities, calf pain with/without exercise, chest pain/ heart murmur, dyspnea on exersion/syncope, irregular/ rapid heart rate, leg pain/ cramps.

Respiratory:- asthma,COPD/pneumonia/ emphysema, coughing, hoarseness/ obstructive sleep apnea, shortness of breath with exersion, shortness of breath/wheezing, tuberculosis or exposure.

Gastrointestinal:- abdominal pain/ blood in stool, black / tarry stool, bloating/ diarrhea/ constipation, loss of appetite/ heartbun, nausea/vomiting, ulcer diseas/ pain after eating, vomited blood.

Genitourinary:- impotence, incontinence of urine/difficulty in voiding, kidney stones, suprapubic/ indwelling catheter,urgency/ bloor in urine.

Integumentary system:- new skin lesions/ skin cancer, rash/ persistent itching, unhealed/ delayed healing of sores.

Neurological:- migraines/headache/vertigo, temporary/ paalysis arm/ leg /face., tingling / numbness, speech difficulties/ seizures.

Musculoskeletal: Artificial kneeor hip joint, back pain/ joint pain, degenerative/ osteoarthritis, muscle pain / weakness/ cramps , rheumatoid arthritis.

Endocrine:- Cold/ heat intolerance., history of drug resistent infection.

Psychiatric:- Anxiety/depression, confusion/ memory loss, difficulty sleeping.

Heme/ lymphatic/ Immune:- AAnemia/ low platelet count, bleeding disorder/ easy bleeding, easy bruising, lymphoma/ leukemia, frequent illnesses.
