Provide a reflection on what you have learned as you created your project. What would you have done differently? What worked well, and what did not? (Questions are in regard to a Capstone Project Paper in Healthcare. Specifically on Medication Errors in Emergency department.
In our department previous days it was common in medication error especially with newly joined staff so we decided to study why this is happening when we did project on it by asking this staff and evaluating the result we came to know that there are different factors for medication error in that some of it was the problem with understanding of doctors handwriting the newly joined staff were scared about asking to seniors for discussing with doctors and the second reason was misunderstanding the labels on the medication and the third reason was sometimes doctor advised them to give medication by or orders like medicines can cause other medication 20 what's the the findings we got after conducting a survey
So we find out a solution for all first week we made a policy to confirm the medication with other staff before administering the medication we reduce the medication error by doing this
We also introduced one policy administer medication only after prescription of the doctor, if the doctor is very busy e inform the other junior Doctors and asked them to write an order behalf of treating doctor after discussing with him
For the second policy we introduced doctors should write capital letters in order sheet so that the understanding will be easier and reduce medication error
Also we introduced labelling the medication lookalike and sound alike medication in a separate cupboard with attractive levels so so in an extent we could able to reduce the medication error
What was the difficulty we found during this implementation is is when that when department is very busy it was very difficult to to confirm the medication with other staff because all staff were busy with patients it can be solved by adequate staffing but we need to get permission from the management to hire more staff
And second in a busy setting it is found that writing orders before giving medication is causing delay in administration of medication that was the second not is able incident
