Nursing Care plan for Tracheostomy Care
· Instruct the family members to do hand washing before and after touching the equipment.
· Educate the family regarding the need of a tracheostomy and its purposes.
· Instruct the family members to clean the tube and the around skin once in a day.
· Provide proper instructions regarding the suctioning and caring of the tracheostomy collar.
· Instruct them to clean the suction tube and other equipment’s used for the suctions.
· Advise them to use sterile technique to prevent the risk for infection.
· Instruct the family members to inform the healthcare provider if patient has any signs for difficulties including Shortness of breath, wheezing, Swelling around the trach tube, Fever of 100.4°F (38°C) or high, Yellow, foul-smelling, bloody, or thick mucus etc.
Equipment need in home includes:
· Sterile disposable tracheostomy cleaning kit
· Sterile suction catheter kit
· Sterile normal saline
· Sterile gloves
· Clean gloves
· Towel or drape
· Cotton twill ties
· Clean scissors
