You are preparing a patient for discharge as directed by the nurse preceptor. Aside from the discharge documents, you are not familiar with the patient’s condition. The patient reports having severe pains. List and describe your next three (3) actions. Provide rationale for each action.
We are preparing a patient for discharge as directed by the nurse preceptor. Asides from the discharge documents we are not familiar with patient's condition. The patient reports having severe pain.
In this condition we should take further next three (3) actions/steps:-
Action 1:- Firstly, we should identify the patient and his condition, monitor and record patient's vital signs ( temperature, pulse rate, respiratory rate,spo2) with pain scale monitor.we should provide psychological support to the patient and gain patient's confidence.
Action 2:- We should inform the physician/duty doctor about the patient's condition with patient's vital signs and his discharge procedure which is in process.
Action 3:- Note the doctor's /physician's order and follow the same further actions i.e order the medications to the pharmacy on urgent basis and receive the medicines and then administrator the medications to the patient with proper procedure and monitor the patient's condition frequently.
