Ans: The most difficult questions like based on IQ level, thinking ability, cognitive thingking questions are most difficult questions for the children /mental health/ elderly patients.

In the patients( elderly, children, mental health ), they have low ability to think. Lets see in elderly, the disease like Hypertension, Diabetes can affects cognitive function in elderly persons. So it affects to the task and can't complete very easily with them.same as in children, the function of all the body system delevelops one by one in our human body. The cognitive system which the child can able to think and do the difficult task are not mature like adults.

And in mental health,they are the person which are mentally retarded . It affects to the person in thinking process and not able to response quikly and easily .

The questions for assessing the client should be comfortable question to the patient. And the questioned should in absence of attenders if he/she is not comfortable to answer in from of any attendants.

When we assess the patients we need to know the patients condition, symptoms and comfortable level . This can affects and help in assessing the client for the further treatment.
