Yes i have provided have provided individualized support to a client in the community services industry because we are caregivers to provide skilled care so their loved ones can live longer in their homes—and avoid expensive long-term care facilities.Nurses are also taking on bigger roles in care coordination, care transitions, health wellness, education and promotion, all of which support caregivers.

a. i.) I prepared following sheet to prepare for the support activities :

• Assessment (gather subjective and objective data, family history, surgical history, medical history, medication history, psychosocial history)

• Analysis or diagsnosis (formulate a nursing diagnosis by using clinical judgment; what is wrong with the patient)

• Care Planning (develop a care plan which incorporates goals, potential outcomes, interventions)
