1)Risk and quality management is essential for working Of good health care system. Clinical risk management is concerned with improving the quality and safety of health care services by identifying the situation that put patient at risk and taking appropriate action to prevent the occuranace of those circumstances .The main goal is to identify problems before it happens .

Main problems associated with the risk and quality management is medical errors, malpractice,violation of patient rights, etc,

Main challenges of risk and quality management

:inappropriate decision making of health care personal ___all health care personal in a health care organization put a major role in providing quality health care service to clients. The quality care is depend on the decision making skills of health care person. Health care organization should take care of develop a structure of risk decisions making.

:lack of risk assessments process___the poor process of risk and quality management will lead to failure to identify the problem of health care system it will put patient at risk.

:social issues___cultural and social issue is one of the challenge to risk and quality management. Health care system failed to provide care based on culture of patient it will lead to increased patient compliance.

:failure to manage risk at correct time. ___if the organization are fail to manage risk at correct time it will lead to further provision of poor quality health care services.

:increase cost of health care___in modern era the cost of health care is day by day increased, it will negatively affect the risk and quality management.

:health care staffing shortage___it is important to have adequate number of health care personal according the ratio of patient to provide high quality health care. This is one of the challenge in risk and quality management. Shifting of duty of health care personal is one of the problem for patient safety.

:slow adoption of information technology ___this is one of the challenge in risk and quality management .

2)in my opinion the challenge that is hard to overcome is health care staffing shortage and frequent changing of duty of staff for same patient. The normal health care system composed of health care personal and patient. Adequate number of health care person is necessary to provide prompt care to patients. Now a days the staff shortage is more and it seems to difficult to handle this problem and more than one staff is caring for one patient is also a problem. It will lead to patient dissatisfaction and poor quality care. In the present situation we can not provide one staff for one patient for total care of patient.
