The alternative medications for patients who have allergy to penicillin can take the following drugs,
Fluoroquinolones - example is ciprofloxacin, ofloxacin.
Tetracyclines - eg. Doxycycline, minocycline.
Macrolides like azithromycin, clarithromycin.
Aminoglycosides like gentamycin.
Glycopeptides like vancomycin.

For sinusitis for a 20 year old client with sickle cell disease,

Nasal decongestant - oxymetazoline nasal drop or saline nasal drop can be used.
Cough suppressants - especially mucolytics are given to soothen the mucus.
Antihistamines like citerezine hydrochloride 5 or 10 mg tablet during night.
NSAIDs ( Non steroidal anti inflammatory drugs) like diclofenac and ibuprofen which are safe drugs to be taken in sickle cell disease.
Analgesic - morphine is the drug of choice to relieve pain in clients with sickle cell disease.
Penicillin as well as other antibiotics can be taken to prevent and stop the growth of bacteria.

Note : some patients may have allergic to penicillin lile drugs but it is safe to use penicillin in sickle cell disease patients who are undergoing treatment for sinusitis.
