- Assess or observe the client for any difficulties he is facing
due to his health condition or as a part of the treatment.
- Yes, if the patient is facing some difficulties (eg: difficulty
in seeing, difficulty in hearing, unable to swallow medications,
difficulty in remembering, etc.) contact the supervisor immediately
and inform regarding the difficulties facing by the patient.
- If the family members are present at the time of assessment,
then talk to them regarding the patient's difficulties.
- Identify how you can help the patient by encouraging the
patient to speak regarding the troubles he is facing and provide a
solution to reduce the problems( eg: providing eyeglasses or
contact lens in vision problems, provide a glass of water to avoid
the dryness in the mouth before medication administration in case
of swallowing difficulty, etc.)
- Document the problems related to the patient and what steps you
followed to minimize it in the chart.
- If the problems still exist, note the patient to follow-up next
visit to see.
