HCOs are strongly oriented toward healing the sick, one person at a time. The first word of the body of this chapter—“patient”—is consistent with that tradition. Consider the following reasons for seeking care:
• Your grandmother, well into her eighties, wants a checkup. She has many of the limitations of age—her hearing, eyesight, and mobility are not what they were, and she has diabetes.
• Your father, 55, has acute chest pain that started a half hour ago and seems to be getting worse.
• You (or your partner) might be pregnant.
What constitutes an excellent result for each of these events? What care teams and clinical support teams are likely to be involved? How do the strategic teams help the clinical teams achieve excellence? If the HCO delivers that excellence, what other factors would be important to continued good health?
The best way to solve for these three types of situation are
1) we have to give priority to the patient with chest pain. An immediate care is essential at this time to save the patients life. As the patient is suffering from accute ches pain for about 30 mins it can lead to irreversible cardiac damage and severe complication that may lead to death.
*Clinical support and care for chest pain includes
1) provide oxygen to the patientkeep sats over 94%
2) if the pain is severe consider giving iv opiods and anti emetics.
3)Further treatment depends on cause the cause of chest pain
2)As we know pregnancy is a part of women's life with two lives in her,so there is a need of compulsory care has to be done without making the patient wait for a long time.
* Clinical support and care includes
-Regular check up for
• every 4 weeks up to 28th week of pregnancy
• every 2 weeks until 36 weeks
• once a week until delivery
3)The elderly women in her eighties with age related problems can be cared by the HCO as she needs regular health check up.
*Clinical support and care includes
• Support with medication
• Collect medicines and refill prescriptions
• Support to attend general practitioner, visit hospital and other medical appointments
The strategic team help the clinal team in following ways _
*Proper risk management.
*Education and Training to handle emergency
* Ensure Patient and Public Involvement.
* Proper staff management by providing experienced staff during emergency.
* Reduce the variation in HCO performance
* Meeting the requirements of clinical care teams
* Close, quantitative monitoring and evaluation
*Leadership rounding
Other factors include
.*Timely management of emergency situation Maintain good public health , p balanced diet exercise, long term care ,proper check up by physician
