If I face an adverse events then I would like to use formal type of reporting as an emergency basis. As any kind of miscommunication can occurs on this kind of setuation in between the stuffs. To prevent unnecessary bias I must choose the formal reporting method.

Like a setuation arise - A patient admitted ina hospital for a long time in a bedridden setuation. Pressure ulcer and hospital acquired pneumonia arises to him. His family members claim that this occurs due to inappropriate supervision and careless job of the health care providers.
