Ms. Hamlet has been a patient with Dr. X for 5 years. Dr. X has provided Ms. Hamlet with excellent care and suggestions of how to improve her health. Ms. Hamlet has continued refused to adopt the recommendations that Dr. X has provided for her on a 6 month evaluations.

Because of the lack of concern that Ms. Hamlet has shown for her own health, Dr. X has decided to terminate Ms. Hamlet as a patient from the practice. Dr. X is very concerned that Ms. Hamlet cardiac status is failing. She has refused to take her medication, exercise, eat a well balanced diet.
