Ms. X., aged 55 years, has been complaining of severe fatigue and “indigestion.” Her son is quite concerned and decides to take her to the emergency department. On arrival she appears very anxious, and her facial skin is cool and clammy; her blood pressure is 90/60, and the pulse is around 90, weak, and irregular. She is given oxygen, an intravenous line is opened, and leads for ECG are attached. Blood is taken for determination of serum enzymes and electrolytes. Tentative diagnosis is myocardial infarction involving the left ventricle. Her son provides information that indicates Ms. X is a long-time smoker, has a stressful job as a high school teacher, is recently separated after 20 years of marriage, and is fearful of losing the family home. She has also seemed to be more fatigued and stopped going to the gym about 18 months ago. She has begun to rely on “fast foods” like pizza and fried chicken and cooks infrequently. Her father had died of a heart attack at age 50. She had also noticed more fatigue and intermittent leg pain when walking or climbing stairs at work. Generalized atherosclerosis is suspected.

Causes of myocardial infraction. From atherosclerosis is ,when arteries become narrow or hardened due to cholesterol plaque build up.further narrowing may occur from blood clots that form on the surface of plaques.
Here patient felt it as a chest pain not a indigestion. It is a pain or discomfort caused when our heart doesn't get enough oxygen rich blood to the muscles of heart.it may feel like squeezing in the chest.discomfort also can occur in the shoulders, arms,neck ,jaw,or back.it may even feel like indigestion.
Here she had atypical symptom that is indigestion .it is frequently defined as epigastric pain ,burning or stabbing....it may effect the delsyed treatment .because of delayed identification of angina.
Cardiac enzymes can see elevated in this situation myoglobin is released more rapidly than the troponin and CK.other lab values are Na,K,Cl,creatinine, urea electrolytes ate significantly lower and ratios of them are will be higher.
EcG will shows ST elevation in anterior leads.negative T waves and finally pathologic Qwaves devolep.PCVs occur near the peak of the T wave of the preceding beat.this may lead to ventricular tachycardia, or fibrillation .R on T or long Q - T intervals can seen in attack patients .Life style changes in heart attack to prevent next episodes.
Stress in a persons life that may effect the risk factors of heart disease. Stress induced cardiac myopathy is not associated with the artery blockages that lead to heart attack.so free from stress is very important to prevent MI.
Cardiogenic. Shock can be caused by reduces blood flow, low blood volume, (as beacuse of dehydration or bleeding.)changes in blood vessels related to infection or allergic reactions.
Here this scenario says she is less stable,fatigue and she is looks pallor. Here the reason may be low blood volume. In this situation heart cannot pump enough blood and oxygen to brain ,kidney's, and other vital organs.
If occurs decompensated shock there will have problems in falling blood pressure, tachycardia, tachypnea,low urine output and labored and irregular breath.in this situation body tries to compensate by limiting blood flow to the extremities the hands and feet causing them cool down .
