How will you prepare for your duties and/or collaboration with the responsible parties regarding care coordination and case management for the OT clients to whom you will be assigned during your first Level II rotation at the assigned site? What elements of care coordination and case management do you think will be particularly challenging? What strategies will you use to meet these challenges? Also consider how you would plan to utilize management structures and supervision at your assigned Level II facility to successfully work with clients to address their issues related to care transitions and life transitions.
OT clients come across different healthcare professionals who involved in OT Procedure. Introduce yourself and explain the procedures in detail to relieve the fear and anxiety of the patient. Encourage the OT staff to motivate the client so the maximum cooperation can be obtained. Go through the patient's medical records, documentation, lab reports, and other related documents to prevent errors and to promote the well-being of the client. Choose the time as per the patient's wishes. Motivate for the therapy if needed. Talk with the family members and provide reassurance. Set clear goals for the Treatment procedure. Meeting the physical and psychological needs of OT clients is challengeable.
Care coordinator must Perform activities in between two or more healthcare professionals to delivery appropriate client care. Develop an effective relationship in collaboration with the patient care. Involve the family members in the care coordination. Establish the care team. Ensure the consent has taken and the routine medical investigations are done. Ensure the managed care plans, primary care services are arranged properly.
Case coordinator has to ensure the progress of the client. Identify the self-management support team of the clients. Make the clinical coordination of the patient care. Use Universal release of information in case of substance abuse or disorders. Make the standard protocols. Develop the workflow process. Utilize the effective communication with the multidisciplinary team. Meet the special care intervention for the needed clients. Coordinate and monitor services for individuals and evaluate the care coordination and outcomes.
